New Patients
Welcome new patients! We are conveniently located on Eastlake Avenue and minutes from downtown Seattle, offering easy accessibility to our patients. There is always ample street parking right outside our offices, and best of all, it’s free!
Start your appointment with a cup of hot tea or beverage of your choice, and prepare your body for rejuvenation! If it is your first time seeing us, you will need to fill out new patient paperwork. You can, however, fill out these forms prior to your appointment.
Please download the new patient form and bring it to your appointment already completed and we'll get you in and out in no time
Insurance and Billing
Remember to bring your insurance ID card to your first appointment. Alliance is a preferred provider for most insurance plans and we'll be happy to check your benefits and bill your insurance directly.
